Start by completing the online enrollment form. Please provide accurate and complete information so our team can process your registration.

 STEP 2 - After submitting your enrollment form, complete your required payment to secure your seat in the upcoming class.

Once your enrollment and payment have been received, ELIFE Medical Academy will email you the required enrollment agreements and additional instructions to complete your registration.

Please use the same name and email address during payment that you entered on your enrollment form.
